Initial Consultation

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Endodontic treatment is usually completed in 1-3 treatment visits. Sometimes, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an in depth evaluation and a subsequent appointment(s).

Please provide the following information at the time of your consultation:

  • Your consultation referral slip and any X-rays if applicable
  • A list of medications you are presently taking
  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • If possible, please preregister with our office by filling out our secure online Patient Registration Form and click the Submit button at the bottom to automatically send us your information. If you prefer to bring the completed form with you to the office, simply print the form by clicking the print button.

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation.

Please alert the office if you have a serious medical condition (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Ray

If your dentist or physician has taken x-rays and would like us to review them, they may forward them to our office. We take digital x-rays at our office for more detailed examination.
